No, marriage is not required for in vitro fertilisation. According to Czech legislation, a written request for treatment signed by both partners – a man and a woman is needed, even in treatments using donated gametes.
Unfortunately, according to the Czech legislation, assisted reproduction is a heterosexual couple therapy. Both partners need to participate in the treatment (but their presence is not required for all appointments). Only a heterosexual couple, married or not, can thus ask for treatment. Single women and lesbian couples cannot legally access assisted reproduction in the Czech Republic.

The law forbids assisted reproduction techniques to be used in sex selection, except for cases where a sex-linked genetic disease is to be avoided.
